Is it Possible to Provide Resident's Training with Real Surgery without Risking Patients Safety and Percuta­neous Nephrolithotomy Efficacy?

Özet

Introduction & Objective: To evaluate the outcomes of per­ cutaneous nephrolithotomy (PCNL) performed by single resi­ dent under the supervision of an experienced endourologist. Methods: Between January 2017 and 2018, outcomes of patients (Group l) who underwent PCNL with an accompanying en­dourologist were retro-prospectively investigated. The stone free and complication rates of these procedures were compared with results of PCNL's performed by endourologists (Group 2) in same center.

Results: A total of 62 patient records were involved into the study. Thirty patients were in Group 1 and 32 patients were in control group (Group 2). There was no statistically difference between two groups according to stone free (Group l; 87%, Group 2; 92%) and complication rates (Group 1; 23%, Group 2; 22%) (p=0.7 and p=0.89, respectively).

Conclusions: PCNL is most challenging technique to teach in residency. Although validated virtual simulators are grown up, reaching this models is not always possible. However, acquiring surgical skills on real surgery during apprenticeship training is effective and safe method and provides steep learning curve in PCNL.
