Circumumblical incision is shown to be an alternative to the usual right upper quadrant horizontal incision of infantile hypertrophic pyloric stenosis because of its cosmetic superiority. In this study a retrospective evaluation of 44 cases in which two different incisions were used between 1992 to 1995 in our clinics. There is no difference regarding preoperative preparation and postoperative follow-up and feeding protocols between the groups. Circumumblical incision was used in 21 and right upper quadrant horizontal incision was used in 23 cases. No statistically significant difference was observed in age of admission, first symptom time, the time of fluid-electrolyte therapy and total time of hospitalization between two groups (p>0.05). No serious complication was seen during and after the operations. Although a second incision was needed in one of the early cases, with the development of experience no such problem was seen again. We think that circumumblical incision gives a good cosmetic result without obvious technical difficulties.
