Metal–organic frameworks (MOFs) exhibit remarkable design versatility, precise pore architectures, responsiveness to external stimuli, high surface area, and adjustable morphology. This review presents a comprehensive summary of MOF-based nanocarriers for drug delivery developed over the past decades, emphasizing essential structural features. We also examine various modifications that can enhance or redirect MOF functionality to improve targeting efficiency or reduce off-target effects. Overall, this review offers researchers valuable insights into recent progress and ongoing challenges in the field of MOF-based drug delivery systems.
