ÖZ
Tıbbın uğraş alanında kullanılan teknolojilerin daha uzun, daha sağlıklı ve daha “kusursuz” bir yaşam sunarken, adil dağıtımı, etik açıdan haklı çıkarılması ve törel değerlere uygunluğu sorunsal oluşturmuştur. Sağlık hizmetlerinde teknoloji uygulamalarının bahsi geçen çift etkili mahiyeti “sağlık teknolojisi değerlendirmesi”ni (STD) elzem kılmıştır. Batı’da “tıbbi teknoloji değerlendirmesi” düşüncesiyle 1960’lı yılların ortalarında temelleri atılan STD’nin zamanla kapsamı genişlemiş ve içeriği de geliştirilerek, günümüze kadar uygulanagelmiştir. STD ile kullanılan teknolojilerin uzun ve kısa erimde etkililik, güvenlik, maliyet, hukuk ve etik boyutlarıyla hasta, kurum ve toplum açısından irdelenmesi esas alınmıştır. Aynı zamanda zararın önlenmesi, faydanın yükseltilmesi, insan onuru ve haklarının korunması, adaletin temin edilmesi, özerklik, mahremiyet ve gizliliğe saygı duyulması da hedeflenmiştir. Şeffaflık, mahremiyet ve gizliliğin korunması, hasta ve toplumun “en üst yararı”nın sağlanması, eşitlik ve adaletin olanaklı kılınması STD’nin değerlendirme boyutlarında ve uygulanmasında salık verilen temel ilkeler olarak belirlenmiştir. STD’nin etiğe içkinliği bir diğer ifadeyle özünde bulunan ve kalıcı olan ahlaki özelliği, mahiyeti ve temel ilkelerinde görülebilmektedir. Dolayısıyla STD’nin, sağlık teknolojisinin “sosyal ve ahlaki norm ve değerlerle ilişkisinin incelenmesi” anlamına gelen “etik analiz”ine gereksinim ortaya çıkmaktadır. Zira etik analizin karar vericilere ve sağlık politikası üreticilerine yardımcı olması bağlamında da önemi yadsınamaz niteliktedir. Etiği, STD’nin “içkin bileşen”i olarak değerlendiren ve tanıtlamak amacıyla yapılan bu çalışmada; sağlık hizmetlerinde teknoloji kullanımının tıbbın değerlerine uygunluğu, etikle bütünlüğü ve ortaya çıkan değer sorunlarına etik açıdan haklı çıkarılabilir çözüm bulunması bağlamında STD’nin içeriği irdelenmiştir. STD’nin mahiyeti, değerlendirme boyutları ve temel ilkeleri incelenerek, her bir aşamasında etiğe içkinliğinin ve etik analizin öneminin ortaya konulması hedeflenmiştir.

ABSTRACT
While the technologies used in health care offer a longer, healthier and more “impeccable” life, they also lead to a paradox within the context of fair use, ethical justification and compliance with moral values. The above-mentioned dual-impact quiddity of technology applications in health care made “health technology assessment” (HTA) necessary. HTA, which was based on the approach of “medical technology assessment” in the mid-1960s, in the West, expanded over time and has being conducted today since its scope has been expanded and its content has been improved. It was grounded on short- and long-term consequences of use of technology in terms of effectiveness, safety, cost-effectiveness, law and ethics with regard to the patient, institution and society by means of HTA. Furthermore, it was aimed at preventing harm, enhancing benefit, protecting human dignity and rights, ensuring justice and respect for autonomy, privacy and confidentiality. Transparency, preservation of confidentiality and privacy, ensuring the “highest benefit” of patient/society, enabling equality and justice were designated as the “key principles of HTA” recommended at the aspects and conduct of HTA. The immanence of HTA to ethics, in other words the moral character of being as a natural and permanent part of HTA, can be noticed in its essence and key principles. Therefore, there is a need for “ethical analysis”, which briefly means to “examine the relationship between technology and social-moral norms and values”, of health technology in order to reveal that ethics is the “immanent component” of HTA. Importance of ethical analysis within the context of supporting decision-makers and health policy-makers is incontrovertible. In this study, the role of HTA was scrutinized in sense of compatibility of technology use in healthcare with values of medicine, its ethical integrity and finding ethically justified solutions for the value issues arising from the use of technology. It was aimed to unveil the immanence of HTA to ethics at each stage and through the process by analyzing the essence, aspects, key principles and ethical analysis of HTA.
