Free functional muscle transfer is a well-established technique for dynamic facial reanimation; however, the inset phase remains technically demanding due to limited operative space and time constraints related to ischemia. We describe a series of technical refinements focusing on muscle preparation, suture organization, and inset strategy. Key elements include determining muscle length in the neutral facial position, use of preplaced multilayer fixation sutures, creation of a load-distributing mesh at the muscle ends, and systematic organization of sutures using numbered instruments to prevent entanglement in a restricted field. A double-loop fixation technique is utilized at the oral commissure, followed by secure proximal anchoring to the deep temporal fascia. These refinements facilitate efficient inset within a narrow (3-4 cm) operative field, reduce suture confusion, and allow controlled, reproducible fixation under limited ischemia time constraints. Structured suture management and optimized inset techniques improve operative efficiency and may enhance the reliability of functional outcomes in free functional muscle transfer for facial reanimation.
