Empty follicle syndrome indicates the absence of oocytes at the time of aspiration, despite normal serum estradiol (E2) levels and follicular development after controlled ovarian hyperstimulation in assisted reproductive technology (ART). Our case is an empty follicle syndrome case that HCG administration was not administered by herself at all. This condition was recognized during aspiration of one ovary. Then aspiration was stopped and HCG was readministered, 36 hours later aspiration was repeated from other ovary and 11 oocytes were retriewed. Transfer of 5 embryos has resulted in a twin pregnancy.
