Giant popliteal artery aneurism

Özet

Popliteal artery aneurysms (PAA) are the most common (0.1-2.8%) peripheral artery aneurysms. Although popliteal artery aneurysms are rare, they can cause serious complications. One of the reasons for the surgical treatment of PAA is thromboembolism and its complications and another reason for surgery is to prevent rupture by enlarging the aneurysm itself. 82-yr male patient admitted to the emergency service with the development of sudden pain, coldness and paleness in right lower extremity. The patient was taken into an emergency operation, as a PAA with a size of 8.6x5.5x4.4 cm was observed in the Doppler USG and distal flow was not detected due to thromboembolism. PAA was exposed through an incision applied to the popliteal fossa with a posterior approach. The aneurysm was fully explored, embolectomy was performed on anterior tibial artery (ATA), peroneal tibial artery (PETA) and posterior tibial artery (PTA) with a fogarty catheter. Then, the aneurysm was excised and 10 mm polytetrafluoroethylene (PTFe) graft was placed in its place. The proximal part distal to the popliteal artery and the peronal truncate were anastomosed end-to-end. ATA was anastomosed to graft with an end-to-side anastomosis. Postoperative pulses were palpable in the dosalis pedias artery and PTA. Limb loss was demonstrated at rate of 20-59% and mortality rate 5.4-11.8% in patients who underwent emergency surgery with critical ischemia. Surgical approach is performed posteriorly or medially. Endovascular approaches are also available, but longterm results of endovascular approach are required, as stent fractures secondary to flexion movement of the knee joint may occur.
