Background: We aimed to obtain the first results of this new method applied by anaesthesiologists in our clinic which involves the combination of the nerve blockade and field infiltration. Methods: An ilioinguinal and iliohypogastric nerve block and an infiltration of subcutaneous tissue and fascia with a spinal needle were applied to a group of 30 ASA I-III patients who were to undergo unilateral inguinal hernia surgery (LA). The traditional spinal subarachnoidal anaesthesia (SA) method was applied to 20 patients. During the surgery pain, the necessity for supplementary analgesic and local anaesthetic, the local tissue factors and the next day patient satisfaction were observed. Results: During the surgery, 6 patients (6/30) of the LA group felt pain, whereas 4 patients (4/20) of the SA group felt pain. 2 patients from the LA groups switched to general anaesthesia due to surgical reasons and one patient due to the failure of the local anaesthesia. The remaining 3 patients who felt pain were treated by supplementary fentanyl and local anaesthetics. 3 patients from the SA group were given supplementary fentanyl. The following day of the surgery, the patients of group LA all expressed that they were pleased by the method; whereas 4 patients in group SA expressed that they were unhappy by the method. Conclusion: This new method that can be applied by an anaesthesiologist, by which the tissues are not traumatized, no haemetoma observed, oedema doesn't cause any discomfort and gains time for the surgeon and satisfies the patient.
