Sünnet sonrası pansuman yapılmalı mıdır?

Özet
Amaç: Sünnet sonrası yapılan gazlı bez veya sentetik bant (SB) pansumanlarının gerekliliğiniaraştırmaktır.Yöntem: Çalışmaya mart 2016 - ağustos 2018 arasında sünnet uygulanan 150 erkek hasta dahiledildi. Hastaların kurumsal veri tabanlarındaki bilgileri retrospektif olarak değerlendirildi. Bu verilerdoğrultusunda hastalar üç grupta incelendi. Grup 1’e sünnet sonrasında SB ile pansumanyapılan hastalar, Grup 2’ye sünnet sonrasında gazlı bezle pansuman yapılan hastalar, Grup 3’esünnet sonrasında yaraya sadece antibiyotikli merhem uygulanan hastalar alındı. Hastaların sünnetsonrası takiplerinde enfeksiyon, kanama, pansuman açılırken yaşadıkları ağrı ve diğer verilerideğerlendirildi. Bulgular gruplar arasında karşılaştırıldı ve istatistiksel olarak analiz edildi.Bulgular: Grup 1’de 60 hasta, grup 2’de 30 hasta ve grup 3’te 60 hasta vardı. Grup 1, 2 ve 3’tesünnet sonrasında müdahale gerektirecek kanama yoktu. Pansuman açıldığı sırada grup 1’dehastaların %5’inde, grup 2’de ise hepsinde ciddi ağrı gelişmişti. Ayrıca, grup 1’de bir hastada, grup2’de üç hastada pansuman açıldıktan sonra kompresyon gerektiren kanama olmuştu. Grup 1’deSB’ın kalış süresi ile enfeksiyon gelişmesi arasında istatistiksel olarak anlamlı ilişki saptandı(p<0,001).Sonuç: Sünnet sonrası gazlı bez veya sentetik bant gibi malzemelerle pansuman yapılmasınagerek olmadığını düşünmekteyiz. Eğer SB ile pansuman yapılacaksa pansumanlar en fazla iki güntutulmalıdır.
Objective: To investigate the necessity of wound dressing with gauze or synthetic bands (SB) after circumcision. Method: A total of 150 patients circumcised between March 2016 and August 2018 were included in the study. The data of the patients in the institutional database were evaluated retrospectively. Three groups of patients were examined in parallel with these data Groups 1 and 2 included patients whose wound dressings after circumcision were performed with SB and gauze, respectively. Group 3 included patients whose wounds after circumcision were dressed only with antibiotic ointment. Infection, bleeding, pain and other complications were evaluated and compared among groups. Results: There were 60 patients in Group 1, 30 patients in Group 2 and 60 patients in Group 3. There was no bleeding requiring intervention after circumcision in all groups. At the time of undressing, 5% of the patients in Group 1 and all of the patients in Group 2 had severe pain. In addition, in one patient in Group 1, and three patients in Group 2 had bleeding requiring compression after the dressing was removed. In Group 1, there was a statistically significant correlation between the length of stay of dressing and the development of infection (p<0.001). Conclusion: In our opinion, dressing with gauze or synthetic tape is not necessary after circumcision. Dressings should be kept for a maximum of two days if SB was used for dressing.
