Güncel Tanı ve Tedavi Yaklaşımlarının Orbital Kütlelerin Histopatolojik Dağılımına Olan Etkisi: Üçüncü Basamak Referans Merkez Deneyimi

Özet
Amaç: Görüntüleme, tanı ve tedavi yöntemlerinin gelişmesi ile oftalmologların orbital kütlelerde orbitotomi endikasyonları eski yıllara göre değişkenlik göstermiş olabilir. Ancak literatürde değişen orbitotomi endikasyonlarının orbital kütlelerin histopatolojik dağılım oranlarına bir etkisi olup olmadığını araştıran bir çalışma son yıllarda yapılmamıştır. Bu nedenle çalışmamızda, orbital kütle nedeniy- le orbitotomi uygulanan hastaların histopatolojik özelliklerini ve oranlarını değerlendirmek amaçlanmıştır. Gereç ve Yöntem: Ocak 2011- Şubat 2019 tarihleri arasında orbital kütle nedeniyle ardıl olarak orbitotomi uygulanan hastalar geriye dönük olarak değerlendirildi. Alınan materyaller histopatolojik inceleme sonucuna göre kistik, vaskülojenik, periferik sinir, optik sinir veya meningeal, fibrokistik, osseöz veya fibro-osseöz, lipokistik veya miksoid, miyojenik, lakrimal bez, metastatik, lenfoid veya lökemik lezyonlar olarak analiz edildi. Bulgular: Çalışmaya dahil edilen 102 hastanın 37’si (%36,2) erkek, 65’i (%63,8) kadın idi. Ameliyat sırasındaki yaş ortalaması 41,3±22,6 (1-79) yıl idi. 90 hastada transkutanöz yaklaşım tercih edilirken, 12 hastada transkonjonktival yaklaşım tercih edildi. Orbital kütlelerin %64,7’si ekstrakonal iken (66 hasta), %35,3’ü intrakonal (36 hasta) idi. Olgulardan alınan kütlelerin patolojik değerlendirmesinde %24,5’i malign (25 hasta), %75,5’i ise benign (77 hasta) karakterde idi. Histopatolojik inceleme sonucuna göre, sırasıyla en yüksek oran- da izlenen gruplar lakrimal bez (%34,3), vaskülojenik (%30,3) ve kistik (%10,7) lezyonlar idi. Lakrimal bez grubundaki hastaların 14’ü epitelyal lezyon iken, 21’i nonepitelyal lezyon idi. Vaskülojenik grupta en sık kavernöz hemanjiyom, kistik grupta ise en sık dermoid kist izlendi. Sonuç: Görüntüleme, tanı ve tedavi yöntemlerinin gelişmesi ile orbital kütlelerde orbitotomi endikasyonları daha önceki çalışmalara gö- re değişkenlik göstermiş olup, histopatolojik olarak lakrimal bez kökenli kütlelerin sıklığı artmış iken kistik grupta azalma izlenmiştir. Bu çalışmada açıklanan farklı orbita kütlelerinin sıklığı hakkında bilgi sahibi olmak oftalmologların olası orbital kütlesi olan hastayı daha iyi yönetebilmesi ve tedavi edebilmesini sağlamasına yardımcı olabilir.
Objective: Orbitotomy indications in orbital masses may have changed compared to previous years with the improvement of imaging, diagnosis and treatment methods. However, there is no study conducted in the recent years investigating the effects of varying orbito- tomy indications on the histopathological distribution ratios of orbital masses in the literature. Therefore, the aim of this study was to evaluate the histopathological features and rates of patients who underwent orbitotomy for orbital mass. Material and Method: Patients who underwent orbitotomy for the management of orbital mass consecutively between January 2011 and February 2019 were evaluated retrospectively. The materials were analyzed as cystic, vasculogenic, peripheral nerve, optic nerve or meningeal, fibrocystic, osseous or fibro-osseous, lipocystic or myxoid, myogenic, lacrimal gland, metastatic, lymphoid or leukemic le- sions according to the histopathological analysis. Results: One hundred two patients with orbital tumors included in the study, 37 (36.2%) were male and 65 (63.8%) were female. The mean age at surgery was 41.3 ± 22.6 years (1-79 years). While the transcutaneous approach was preferred in 90 patients, the transconjunctival approach was preferred in 12 patients. While 64.7% of orbital masses were extraconal (66 patients), 35.3% were intraconal (36 patients). In the pathological evaluation of the masses, 24.5% were malignant (25 patients) and 75.5% were benign (77 patients). According to the histopathological analysis, the groups with the highest incidence were lacrimal gland (34.3%), vasculogenic (30.3%) and cystic (10.7%) lesions, respectively. While 14 of the patients in the lacrimal gland group were epithelial lesions, 21 were nonepithelial lesions. Cavernous hemangioma was most common in the vasculogenic group, and dermoid cyst was the most common in the cystic group. Conclusions: The indications for orbitotomy in orbital masses have changed compared to previous studies with the development of imag- ing, diagnosis and treatment methods. Histopathologically, the rate of lacrimal gland group increased, while a decrease was observed in the cystic group. Knowledge of the distribution of the different orbital masses reported in this study may help ophthalmologists better management of patients with possible orbital masses.
