Abstract

Contact of the skin and subcutaneous tissues with a heat source is called a burn. Burns are

one of the most common injuries seen in emergency departments around the world. Any heat

source can cause burns. In general, approximately 86% of burns are thermal, 4% are electrical

and 3% are chemical burns. Burns cause both local and systemic effects. Various factors play

a role in the severity of the burn, such as the type of burn, the width of the burn, the depth of

the burn, the location of the burn, the age of the patient, the co-morbidities of the patients,

the presence of trauma along with the burn, and the presence of inhalation injury. Burns are

treated at the scene, in emergency rooms, in the hospital or in the burn unit, depending on the

degree of severity. Mild or minor burns can be treated on an outpatient basis in the crime scene

or emergency room setting with cooling, cleaning, closure and pain management methods. The

first step in the treatment of severe burns is to manage burn shock with aggressive but adequate,

but not excessive, fluid resuscitation. As a result, serious burns are injuries that have a multifaceted

and complex clinical course and require clinical stabilization processes such as early

resuscitative, intensive wound care treatment and even surgical wound management, and may

subsequently require long-term physical and psychological rehabilitation

Cilt ve cilt altı dokularının bir ısı kaynağı ile temasına yanık denir. Yanıklar tüm dünyada acil servislerde

yaygın görülen yaralanmalardan biridir. Herhangi bir ısı kaynağı yanığa neden olabilir.

Genel olarak yanıkların yaklaşık %86’sı termal, %4’ü elektriksel ve %3’ü kimyasal yanıklardır.

Yanıklar hem lokal hem de sistemik etkilere neden olur. Yanığın şiddetini yanığın türü, yanığın

genişliği, yanığın derinliği, yanığın yeri, hastanın yaşı, hastaların ko-morbiditeleri, yanıkla beraber

travma olması ve inhalasyon yaralanmasının olması gibi çeşitli faktörler rol oynar. Yanıklar

ciddiyet derecesine göre olay yerinde, acil servislerde, hastanede veya yanık ünitesinde tedavi

edilir. Hafif derecedeki veya küçük yanıklar soğutma, temizleme, kapatma ve ağrı yönetimi

yöntemleri ile olay yeri ya da acil servis ortamında ayaktan tedavi edilebilir. Ciddi yanıklarda

tedavide ilk basamak yanık şokunun agresif ama yeterli, ancak aşırı olmayan sıvı resüsitasyonu

ile yönetilmesidir. Sonuç olarak ciddi yanıklar ise çok yönlü ve karmaşık klinik seyre sahip olup

erken resusitatif, yoğun yara bakımı tedavisi hatta cerrahi yara yönetimi gibi klinik stabilizasyon

süreçleri gerektiren ve sonrasında da uzun süreli fiziksel ve psikolojik rehabilitasyon gerektirebilen

yaralanmalardır.
