Hidradenitis suppurativa tanılı hastaların hurley evrelerine göre retrospektif değerlendirilmesi

Özet

Amaç: Bu çalışmada kliniğimizde hidradenitis suppurativa takip polikliniğine başvuran,

klinik bulgularıyla hidradenitis suppurativa tanısı almış hastaların Hurley evrelerine göre

demografik bilgilerinin, sistemik antibiyotik, adalimumab kullanımının retrospektif olarak

değerlendirilmesi amaçlanmıştır.

Gereç ve Yöntem: Bu retrospektif çalışmaya, İstanbul Üniversitesi-Cerrahpaşa, Cerrahpaşa Tıp

Fakültesi Deri ve Zührevi Hastalıkları Ana Bilim Dalı hidradenitis suppurativa takip polikliniğine

01.01.2018-15.06.2022 tarihleri arasında başvuran ve HS tanısı almış olan hastalar dahil edildi.

Hastaların HS polikliniğindeki kayıtlı verileri retrospektif olarak taranmıştır.

Bulgular: Bu çalışmaya hidradenitis suppurativa tanılı verilerine ulaşılabilen 31 hasta dahil

edildi. (21 erkek, 10 kadın). Erkeklerin yaş ortalaması 36 idi. Kadınların yaş ortalaması ise 30.9

idi. Bu hastalar %19 oranında evre 1’dir. Hastaların %29’u evre 2’dir. Hastaların %51’i ise evre 3’tür.

Evre 1 hastaların %50’si, Evre 2 hastaların %55’i, Evre 3 hastaların %62’si, sigara kullanmaktadır.

Evre 1 hastalarının vücut kitle indeksi 26.16 iken evre 2 hastalarında 27.66, evre 3 hastalarında ise

31.25’tir. Evre 1 hastalarında sistemik antibiyotik kullanmış olanların oranı yüzde 66 iken evre 2

ve evre 3 hastalarının tamamı sistemik antibiyotik kullanmıştır. Evre 1 hastalarında adalimumab

kullanma öyküsü olan hasta yokken evre 2’de adalimumab kullanma öyküsü olanların oranı

%33, evre 3’te ise bu oran %43 olarak bulunmuştur.

Sonuç: Hidradenitis suppurativa kronik, tekrarlayan, süpüratif bir deri hastalığıdır. Hidradenitis

suppurativa abse, sinüs ve daha sonra skar bırakarak iyileşebilen daha sık olarak kıvrım

bölgelerini tutan bir hastalıktır. Hurley Evre seviyesi arttıkça sigara kullanım oranı ve vücut kitle

indeksi daha yüksek bulunmuştur. Evre 3 hastalarında adalimumab kullanımı evre 2 hastalarına

göre daha yüksek orandadır. Sistemik antibiyotik kullanım oranı evre arttıkça paralel olarak

artmaktadır.
