Özet

PURPOSE: Evaluating myomectomy procedures for uterine fibrioid treatment by means of clinical status , operation quality, pathologic outcomes, complications and impact of myomectomy on fertility. STUDY DESING: We performed this retrospective study by scanning patient files undergoing surgery in Cerrahpasa Faculty of Medicine, Obstetrics and Gynecology Department for uterine fibroid removal during years 1996 and 2005. This study was concluded in between September 1rst, 2005 and January 1rst, 2006. The statistical analysis was performed using Chi-square (Fisher's extract when necessary) and ANOVA method. p<0.05 was statistically significant. RESULTS: In myomectomy operations no malignancy was detected on the pathological examination of the specimen. On the other hand, in 403 cases (95.1 %) uterine fibriod diagnosis was histologically confirmed. 65 patients out of 146 (44.5 %) with myomectomy, who had a history of infertility, has at least one pregnancy proceeding the operation. Mean duration of conception was 24.5±18.7 months (6-72). In the subgroup where a single uterine fibroid was removed, only 29.3 % (24/82) achieved pregnancy (p<0.05). In the group where the size of fibroid was smaller than 10 cm in diameter, the conception rate was 51.4 % (54/105), where as when larger than 10 cms, the conception rate was 26.8 % (11/41) (p<0.05). In 20 pregnancies concieved after myomectomy, 16 out of 20 resulted in abortus before 20 weeks of gestation. (22.5 %), but 41 cases gave birth at term.(57.7 %) 7 of the myomectomy patients required secondary operation (1.9 %). One patient out this 7 has undergone abdominal hysterectomy and the remaining 6 had undergone myomectomy. CONCLUSION: In myomectomy cases where more than oneflbriod was removed and larger than a certain diameter, the fertility rates were not as good as planned. Therefore, careful selection of myomectomy patients can predict the fertility rates.
