Progressive pseudorheumatoid dysplasia (PPRD) is a rare genetic skeletal disorder characterized by progressive degeneration of joint cartilage. This condition leads to joint stiffness and pain without inflammation, significantly affecting the quality of life and physical function of patients. In this case report, we aim to demonstrate that a patient with severe bilateral hip movement restrictions and significant knee extension limitations regained painless and functional joint mobility following bilateral total hip arthroplasty and bilateral distal femoral extension osteotomy. Furthermore, we seek to highlight that in cases where rheumatic diseases are primarily considered, the possibility of a rare diagnosis such as PPRD should also be taken into account.
