SUMMARY:
The effect of penile tourniquet and continuous artificial erection on penile tissues: an experimental study
Aim: It is known that penile tourniquet (PT) causes ischemic damage on erectile tissues which increases with longer durations. This study aims to investigate the histopathological and immunohistochemical effects of the "continuous artificial erection" (CAE) technique on penile erectile tissues, in an experimental model.
Material and Method: Total of 37 rats were randomized into 5 groups. Twenty and 40 minutes of PT alone (20T and 40T) and with CAE (20T&E and 40T&E) was applied in 4 groups. CAE was done via continuous intracavernosal saline injection. Penectomy was performed 3 weeks after the procedures in these 4 groups and directly in the control group. Erectile tissue samples were evaluated via light microscopy and immunohistochemistry (eNOS, CD62E and ICAM-1 staining).
Results: Among our study groups, highest histopathological scores for inflammation, fibrosis, and most significant immunohistochemical findings for endothelial damage were seen in the 40T group. 20T&E and 40T&E groups showed decreased degree of fibrosis and endothelial damage in comparison with PT groups. 20T&E showed no evidence of fibrosis, and all the endothelial markers were preserved in this group. Among binary comparisons between the experiment groups, most significant differences were found between groups 20T&E and 40T.
Conclusion: Findings of this experimental study support that continuous artificial erection via intracavernosal saline injection does not increase the erectile tissue damage that is already caused by PT, but rather decreases it.
