Abstract

OBJECTIVE: To investigate the relationship between preoperative serum levels of both CA-125 and thiobarbituric acid reactive substances (TBARS), as a marker of lipid peroxidation, and stage, histological grade and type of the tumor in patients with epithelial ovarian cancer. STUDY DESIGN: Preoperative serum levels of both CA-125 and TBARS and histopathological characteristics of patients (n=49) who had been diagnosed to have epithelial ovarian cancer were investigated. Age-matched healthy women (n=22) were used as controls. Correlations between these parameters and both stage and grade of the tumor were investigated. RESULT(s): Serum levels of CA-125 and TBARS in stages III-IV were higher than those in stages I-II. Preoperative serum level of CA-125 was found to be lower than 35 U/ml in 50% of the stage I-II patients and in 20% of stage III-IV patients. CA-125 level was significantly higher in the stage IH-IV group in comparison to stage I-II group, and CA-125 was found to be poorly correlated with tumor stage. TBARS level in the stage III-IV group was found to be higher than that in the stage I-II group. It was observed that 83% of grade 1 tumors, 20% of grade 2 tumors and 6% of grade 3 tumors have normal CA-125 levels. There was a significant difference between grade 1 and grade 2 groups, and between grade 1 and grade 3 groups but not grade 2 and grade 3 groups for CA-125 levels. CA-125 was found to be correlated with tumor grade. There was no significant difference for serum TBARS levels among tumor groups with different grades. It was determined that preoperative serum level of CA-125 in serous tumors was higher than that in non-CA-125 in serous tumors was higher than that in non-serous tumors, and, 7% of serous tumors and 40% of non-serous tumors have normal CA-125 values. However, there was no significant difference between serous and non-serous tumors for serum TBARS levels. CONCLUSION(s): We suggest that preoperative serum levels of CA-125 may be helpful for the prediction of tumor stage and grade but it is a more reliable marker for the prediction of serous tumors. Although serum TBARS level may increase in patients with ovarian cancer, it is not related with grade and type of the tumor.
